Direct Billing / Fax Claim: The hospital bills the insurer directly; once discharged, you just sign the claim form and walk out without upfront payments.
Reimbursement: You pay the entire hospital bill upfront out-of-pocket, then submit receipts and medical certificates to get refunded later.
Why reimbursement happens: Often triggered when you claim within the first 1–2 years of a new policy, allowing the insurer time to verify there was no pre-existing condition.
